What are the documents needed for Chinese natives to travel to America for a short period?

My cousin, who is Chinese, wants to travel to America for summer vacation – short term. He is a minor (13 years old) with relatives in America who are citizens (aunt, cousins) and ones who have green cards (uncle, direct grand parents).  What are the documents that he needs to provide so that he can come? How long will this process take?

He needs a B-2 visas .. having relatives in the US will probably make it more difficult and will probably be refused
